Photo and Video Consent and Release Form

I grant to (S.V.O.) the right and permission to copyright and/or publish photographs or home videos of myself, or those minor children under the age of eighteen (18) for whom I am authorized to grant this Release for television broadcast, advertising, internet usage, or any other similar lawful purpose.

 I also g= rant to S.V.O. the right to reproduce, edit and digitize the footage at S.V.O.'s sole discretion, to display and distribute the work in television or any related work sponsored by S.V.O.  without any compensation free and clear of any claim on my part.

 I warrant that:

 1.  The work was created by me or those who assigned all right to me and I am the owner of all right and title in the work.

 2.  All game was taken in accordance with all applicable game laws, rules and regulations.
